The badge constitutes metal ,silver plated medal in the shape of rectangle with dimensions 80x80 mm with the image of the white eagle with beak and talons on the front side (obverse) . There is the crown on its head which is turned right ,wings are developed. The images of Wojciech Korfanty, Ignacy Jan Paderewski, Wincenty Witos, Jozef Pilsudski, Roman Dmowski, Ignacy Daszynski and Jozef Haller are placed around the eagle.
In the left part downwards there is an inscription: 100 –LECIE ODZYSKANIA NIEPODLEGLOSCI (CENTENARY OF REGAINING OF INDEPENDENCE)
On the another side (reverse) in the top part there is a quote from sermon „
O miłości Ojczyzny” (On the love of the motherland) – authorship of Armenian-Catholic Archbishop of Lvov

In the bottom part there is the image of Polish legions.
The badge will be granted by the Chairman of the Council of Ministers or the person authorized by him.
